Sub Acute Care

A Subacute Unit generally includes two types of patients – those who are receiving short-term rehabilitation to recover from an injury or illness, and those with multiple medical problems or illnesses who are receiving care for a longer period of time. Patients in our Subacute Unit here at a Villa Las Palmas receive rehabilitation therapy that acts as a bridge, transitioning patients from SubAcute care back to the community. The focus is on physical and functional improvement and personal independence.

Subacute care is comprehensive care designed for someone who has an acute illness, injury or exacerbation of a disease process. It is goal oriented treatment rendered immediately after acute hospitalization to treat one or more specific active complex medical conditions or to administer one or more technically complex treatments, in the context of a person’s underlying long-term conditions and overall situation.  Subacute care requires the coordinated services of an interdisciplinary team including physicians, nurses and other relevant professional disciplines, who are trained and knowledgeable to assess and manage these specific conditions and perform the necessary procedures, working with each patient to restore health and help him or her achieve maximum independence in activities of daily living.  Subacute care is given as part of a specifically defined program for each individual, It is our teams objective to help each patient reach his or her maximum health and functional ability which includes educating both the patient and their family members to successfully manage their daily routine after discharge.

Villa Las Palmas Healthcare Center is proud to boast the first sub-acute care facility in San Diego County founded in 1997. Michael Torbett, BS/RCP is our Director of Respiratory Therapist and assisted in opening the unit and has been a driving force to keep our Subacute a Five Star facility.
